You can easily make a polka dot cake by taking round candy wafers that you use for melting and candy making and putting them into a store-bought cake. Turn them so that the flat side is facing out, and press them into the icing all over the cake to create a polka dot pattern or random spots. You can also paint a fondant cane with polka dots, or pipe on icing dots on a buttercream cake.
